Payment Options and Fees.
If you do not have insurance or wish not to involve your insurance, you may choose to do self-pay. Contact me to inquire about a sliding scale based on income.
Self-Pay.
I am currently able to accept Medicaid, Blue Cross and Blue Shield of Vermont, CBA Blue, and MVP. Check with your insurance regarding your plan coverage, copayments and deductibles.
If you have an Out-of-Network plan with an insurance I am not able to accept, I am able to offer a Superbill for you to present to your insurance company for reimbursement. Check with your insurance company if this is an option for you.
